The most asked question about acne is concerned

Acne is a very common disease. People who have it tend to have similar kinds of questions about it and its treatment. This section addresses some of the common questions asked by people with acne. Please remember that your dermatologist is always the best source of specific information about your individual health issues, including acne.


Questions and Answer does follows:


1. What causes acne?


The causes of acne are linked to the changes that take place as young people mature from childhood to adolescence (puberty). The hormones that cause physical maturation also cause the sebaceous (oil) glands of the skin to produce more sebum (oil). The hormones with the greatest effect on sebaceous glands are androgens (male hormones), which are present in females as well as males, but in higher amounts in males.


Sebaceous glands are found together with a hair shaft in a unit called a sebaceous follicle. During puberty, the cells of the skin that line the follicle begin to shed more rapidly. In people who develop acne, cells shed and stick together more so than in people who do not develop acne. When cells mix with the increased amount of sebum being produced, they can plug the opening of the follicle. Meanwhile, the sebaceous glands continue to produce sebum, and the follicle swells up with sebum.


In addition, a normal skin bacteria called P. acnes, begins to multiply rapidly in the clogged hair follicle. In the process, these bacteria produce irritating substances that can cause inflammation. Sometimes, the wall of the follicle bursts, spreading inflammation to the surrounding skin. This is the process by which acne lesions, from blackheads to pimples to nodules, are formed.


2. I wash my face several times a day. Why do I still get acne?


Many people still believe that acne is caused by dirty skin. The truth is, washing alone will not clear up or prevent acne. Washing does, however, help remove excess surface oils and dead skin cells. Many people use all kinds of products, including alcohol-based cleansers, and scrub vigorously, only to irritate the skin further and worsen their acne. Washing the skin twice a day gently with water and a mild soap is usually all that is required. However, acne is actually caused by a variety of biologic factors that are beyond the control of washing. For that reason, you should use appropriate acne treatments for the acne.


3. Does stress cause acne?


Stress is commonly blamed for the development of acne. Stress can have many physiologic effects on the body, including changes in hormones that may theoretically lead to acne. In some cases the stress may actually be caused by the acne lesions, not the other way around! If the acne is being treated effectively, stress is not likely to have much impact on the majority of people.


4. I never had acne as a teenager. Why am I now getting acne as an adult?


Usually, acne begins at puberty and is gone by the early 20s. In some cases, acne may persist into adulthood. Such types of acne include severe forms that affect the body as well as the face (which afflict males more than females) and acne associated with the menstrual cycle in women. In other cases, acne may not present itself until adulthood. Such acne is more likely to affect females than males.


There are several reasons for this. As females get older, the pattern of changes in hormones may itself change, disposing sebaceous glands to develop acne. Ovarian cysts and pregnancy may also cause hormonal changes that lead to acne. Some women get acne when they discontinue birth control pills that have been keeping acne at bay. Sometimes young women may wear cosmetics that are comedogenic-that is, they can set up conditions that cause comedones to form.


5. What role does diet play in acne?


Acne is not caused by food. Following a strict diet will not, clear your skin. While some people feel that their acne is aggravated by certain foods, particularly chocolate, colas, peanuts, shellfish and some fatty foods, there is no scientific evidence that suggests food causes or influences acne. Avoid any foods which seem to worsen your acne and, for your overall health, eat a balanced diet--but diet shouldn't really matter if the acne is being appropriately treated.


6. Does the sun help acne?


Many patients feel that sunlight improves their acne lesions and go to great lengths to find sources of ultraviolet light. There is no proven effect of sunlight on acne. In addition, ultraviolet light in sunlight increases the risk of skin cancer and early aging of the skin. It is, therefore, not a recommended technique of acne management, especially since there are many other proven forms of treatment for acne. Moreover, many acne treatments increase the skin's sensitivity to ultraviolet light, making the risk of ultraviolet light exposure all the worse.


7. What is the best way to treat acne?


Everyone's acne must be treated individually. If you have not gotten good results from the acne products you have tried, consider seeing a dermatologist. Your dermatologist will decide which treatments are best for you. 8. What kind of cosmetics and cleansers can an acne patient use?


Look for "noncomedogenic" cosmetics and toiletries. These products have been formulated so that they will not cause acne.


Some acne medications cause irritation or pronounced dryness particularly during the early weeks of therapy, and some cosmetics and cleansers can actually worsen this effect. The choice of cosmetics and cleansers should be made with your dermatologist or pharmacist.


Heavy foundation makeup should be avoided. Most acne patients should select powder blushes and eye shadow over cream products because they are less irritating and noncomedogenic. Camouflaging techniques can be used effectively by applying a green undercover cosmetic over red acne lesions to promote color blending.


9. Is it harmful to squeeze my blemishes?


Yes. In general, acne lesions should not be picked or squeezed by the patient. In particular, inflammatory acne lesions should never be squeezed. Squeezing forces infected material deeper into the skin, causing additional inflammation and possible scarring.


10. Can anything be done about scarring caused by acne?


Scarring is best prevented by getting rid of the acne. Dermatologists can use various methods to improve the scarring caused by acne. The treatment must always be individualized for the specific patient. Chemical peels may be used in some patients, while dermabrasion or laser abrasion may benefit others. It is important that the acne be well controlled before any procedure is used to alleviate scarring.


11. How long before I see a visible result from using my acne medication?


The time for improvement depends upon the product being used, but in almost all cases it is more a matter of weeks or months instead of days. Most dermatologists would recommend the use of a medication or combination of medications daily for 4 to 8 weeks before they would change the treatment. It is very important for patients to be aware of this time frame so they do not become discouraged and discontinue their medications. Conversely, if you see no change whatsoever, you might want to check with your dermatologist regarding the need to change treatments.


12. Would using my medication more frequently than prescribed speed up the clearing of my acne?


No--always use your medication exactly as your dermatologist instructed. Using topical medications more often than prescribed may actually induce more irritation of the skin, redness and follicular plugging, which can delay clearing time. If oral medications are taken more frequently than prescribed, they won't work any better, but there is a greater chance of side effects.


13. My topical treatment seems to work on the spots I treat, but I keep getting new acne blemishes. What should I do?


Topical acne medications are made to be used on all acne-prone areas, not just individual lesions. Part of the goal is to treat the skin before lesions can form and to prevent formation, not just to treat existing lesions. Patients are generally advised to treat all of the areas (forehead, cheeks, chin and nose) that tend to break out rather than just individual lesions.


14. My face is clear! Can I stop taking my medication now?


If your dermatologist says you can stop, then stop--but follow your dermatologist's instructions. Many times patients will stop their medication suddenly only to have their acne flare up several weeks later. If you are using multiple products, it may be advisable to discontinue one medication at a time and judge results before discontinuing them all at once. Ask your dermatologist before you stop using any of your medications.


15. Does it matter what time I use my medication?


Check with your dermatologist or pharmacist. If you were taking one dose a day of an antibiotic, you could probably take it in the morning, at midday or in the evening, although you should pick one time of day and stay with it throughout your treatment. With oral medications prescribed twice a day or three times a day, you should try your best to spread out the doses evenly. Some antibiotics should be taken on an empty or nearly empty stomach. For optimal results with topical treatments, you should strictly follow your dermatologist's recommendations. For example, if instructed to apply benzoyl peroxide in the morning and a topical retinoid at bedtime, it is important to follow these directions strictly. If the two were applied together at bedtime, for example, you could decrease the efficacy of the treatment because of chemical reactions that make them less effective.


16. I have trouble remembering to take my oral medication every day. What's a good way to remember? What should I do if I forget a dose?


This is a common problem. Many patients try to associate taking their medication with a routine daily event such as brushing teeth or applying makeup. It also helps to keep the medication close to the area where the reminder activity is carried out.


In most cases, if you miss a day of your oral treatment, do not double up the next day; rather, get back to your daily regimen as soon as possible--but there may be different instructions for different oral medications. Ask your dermatologist or pharmacist about what to do if you miss a dose of your particular medication.


17. I have been using topical benzoyl peroxide and an oral antibiotic for my acne and have noticed blue-black and brown marks developing on my face and some discoloration on my body. The marks are especially noticeable around acne scars and recently healed lesions. Is this a side effect of medication and is it permanent?


It is not possible to make general statements about side effects of medications that apply to individual cases. A dermatologist should be consulted. The facial marks and body discoloration described by the patient in this case do fall within the range of side effects of some antibiotics.


Unique patterns of pigmentation are sometimes seen in acne patients treated with certain oral antibiotics—particularly minocycline. The pigmentation patterns that appear may include:


* Localized blue-black or brown marks in and around acne scars and in areas of previous acne inflammation


* A "muddy skin" appearance that may cover much of the body


* Diffuse brownish pigmentation of the feet and lower legs.


The pigmentation side effect gradually disappears after the therapy is discontinued.


Any side effect of a medication should be noted by the patient and brought to the attention of the physician. While most side effects are temporary they should be discussed with the physician and monitored.


18. My doctor is prescribing a topical retinoid for my acne. He said a retinoid is a substance related to vitamin A. If the drug is related to vitamin A, shouldn’t vitamin A dietary supplements be helpful in getting rid of acne?


Dietary vitamin A is essential to good health, especially vision. It has healthful effects in the skin. Large doses of vitamin A for the treatment of acne is not recommended on grounds of safety. The retinoids and retinoid-like substances used as topical treatments for acne are prepared especially for their potent effect on the shedding of cell lining in the sebaceous follicle. Their use should be monitored by a dermatologist.


Dietary vitamin A has multiple health effects in the human body. Vitamin A is essential for good vision. Extreme vitamin A deficiency can result in blindness, usually accompanied by dry, scaly skin. Vitamin A overdose that far exceeds the Recommended Dietary Allowance (RDA) of 5,000 IU can have effects nearly as catastrophic. Extreme vitamin A overdose can cause the skin to blister and peel—an effect first seen in early North Pole explorers who nearly died after eating polar bear liver that has an extraordinarily high vitamin A content.


Topical retinoids are usually prescribed as a treatment for moderate to severe acne. Side effects are chiefly dermatologic, including redness, scaling and dryness of the skin, itching and burning. These side effects can usually be managed by adjustment of the amount and timing of retinoid applied to the skin. Dose adjustment must be discussed with the dermatologist who prescribed the treatment.


19. Are there any acne treatments specifically for people with dark skin? Are there any treatments specifically harmful to dark skin?


There are no acne treatments specifically for use on dark skin. Acne treatments are generally as safe and effective on dark skin as on light skin. Some treatments for acne scars may cause temporary lightening of dark skin.


Acne is a common skin disease that has the same causes and follows the same course in all colors of skin.


Very dark or black skin may be less well-moisturized than lighter skin. Topical anti-acne agents such as benzoyl peroxide that have a drying effect on the skin should be used under the supervision of a dermatologist. Benzoyl peroxide also is a strong bleach and therefore must be applied carefully to avoid inadvertent decolorization of a patch of hair, towels or clothing.


Darker skin has a tendency to develop post-inflammatory hyperpigmentation (excessive skin darkening at places where the skin was inflamed). Severe inflammatory acne may result in dark spots. The spots resolve over time; a dermatologist may be able to recommend cosmetic measures to make the spots less apparent until they resolve. Some acne treatments, such as topical retinoids and azelaic acid, may also help fade the discoloration.


Removal of acne scars by dermabrasion or chemical peeling may cause temporary lightening or darkening of dark skin in the areas of treatment. Scar treatment should be discussed with a dermatologist or dermatologic surgeon before it is undertaken.


Alterations of melanin (dark pigments that give the skin its color) pigmentation such as vitiligo and melasma are not related to acne, but they may be present simultaneously with acne. The diagnosis and treatment of melanin pigmentation disorders such as vitiligo requires a dermatologist with knowledge and experience in treating these conditions.


20. Is acne that appears for the first time in adulthood different from acne that appears in adolescence?


Acne has a specific definition as a disease of sebaceous follicles. This definition applies to acne that occurs at any age. However, it may be important to look for an underlying cause of acne that occurs for the first time in adulthood.


Current understanding of the causes of acne vulgaris is described in the Main Text section Why and how acne happens. In brief summary, acne vulgaris develops when excessive sebum production and abnormal growth and death of cells in the sebaceous follicle result in plugging of follicles with a mixture of sebum and cellular debris and formation of comedones (blackheads and whiteheads). Bacteria in the follicles—chiefly Propionibacterium acnes, the most common bacterial colonist of sebaceous follicles—may contribute to the inflammation of acne by release of metabolic products that cause inflammatory reaction. The pathogenic events, which cause disease, in the sebaceous follicle are believed to be due in large degree to changes in levels of androgenic (male) hormones in the body—a circumstance usually associated with growth and development between ages 12 and


21. Some acne investigators believe that although this understanding is generally correct, there is more yet to be learned about the causes of acne vulgaris.


Acne that appears after the age of 25-30 years is (1) a recurrence of acne that cleared up after adolescence, (2) a flare-up of acne after a period of relative quiet—for example, during pregnancy, or (3) acne that occurs for the first time in a person who had never previously had acne.


Acne that occurs in adulthood may be difficult to treat if there are multiple recurrences. Some patients with severe recurrent acne have undergone repeated courses of treatment with the potent systemic drug isotretinoin.


Acne flares in association with pregnancy or menstruation are due to changes in hormonal patterns.


Acne that appears for the first time in adulthood should be investigated for any underlying cause. Drugs that can induce acne include anabolic steroids (sometimes used illegally by athletes to “bulk up”), some anti-epileptic drugs, the anti-tuberculosis drugs isoniazid and rifampin, lithium, and iodine-containing drugs. Chlorinated industrial chemicals may induce the occupational skin disorder known as chloracne. Chronic physical pressure on the skin—for example, by a backpack and its straps, or a violin tucked against the angle of the jaw and chin—may induce so-called acne mechanica. Some metabolic conditions may cause changes in hormonal balance that can induce acne.


Some lesions that appear to be acne may be another skin disorder such as folliculitis—infection and inflammation of hair follicles—that require different treatment than acne. Acne that appears for the first time in adulthood should be examined and treated by a dermatologist.


22. My 15-year-old daughter has what I would describe as a very mild case of acne. She has made it much worse by constant picking and squeezing. She looks in the mirror for hours, looking for some blackhead or blemish she can pick or squeeze. Does she need psychological counseling?


Excessive picking and squeezing of otherwise mild acne is a condition called excoriated acne, seen most often in young women. A dermatologist may provide effective counseling.


The typical person with excoriated acne is a person—often a young women—who is so distressed with her appearance due to acne that she literally tries to "squeeze the acne out of existence." The acne is often very mild, but the person’s face may constantly be covered with red marks from squeezing, and open sores where lesions have been picked open.


The word excoriate means to scratch or abrade the skin. Excoriated acne is a medically recognized condition that should be discussed with a dermatologist. Occasionally giving in to a temptation to squeeze a blackhead is not defined as excoriated acne. Hours in front of a mirror, squeezing and picking every blemish, is a definition of excoriated acne. A dermatologist may be able to counsel the patient regarding a course of treatment in which the patient can participate, but keep "hands off."


23. Can the rate of secretion or the composition of sebum be altered by diet? If it can, shouldn’t alteration of diet be considered a treatment for acne?


Diet has never been proven to have a role in the cause or treatment of acne. Dietary manipulation may have a role in the treatment of some scaling diseases of the skin, but not in the treatment of acne.


Dietary cause is one of the most persistent myths about acne. Foods, such as chocolate or greasy foods, do not cause acne, but certain foods seem to make some people’s acne worse. The following can bring on or worsen it:


* Hereditary factors


* An increase in male hormones found in both males and females


* Menstruation


* Emotional stress


* Oil and grease from cosmetics, work environment


No food has been shown to be effective in preventing or treating acne. A healthy diet is, of course, necessary for good general health.


24. Shouldn't I just try to eliminate sebum from my body?


No. When it isn't blocked in your pores, sebum helps keep your skin healthy.


25. Why does acne usually start at puberty?


No one knows for certain. What is known is that the sebaceous glands that produce sebum get much larger at puberty than they were before.


26. Why does the skin around a pimple turn red?


This redness is caused by the body's inflammatory response. Inflammation is a sign that your immune system is working to fight an infection. However, the inflammatory response doesn't always work perfectly, and can even be the cause of scarring.


27. If my skin turns red, does that mean that I'm going to have scars?


Usually, no. Even when there will be no permanent scar, the aftereffects of the inflammatory response can leave the skin red for months, sometimes for more than a year.

Fold the dough and roll the rolling pin over it, which seals the dough making separate pockets. Make a pattern on the edge of each ravioli, with the help of wimped zig-zag cutting edge. Cook by boiling them till they come to the top of water.

House prices set for greater regional variation

The growth of English house prices is slowing. In contrast, both Scottish and Northern Irish house prices are heading for boom periods, according to new figures.


Nationwide predicts that house prices in Scotland and Northern Ireland are set to become increasingly dislocated from trends in England and Wales, as the countries have witnessed far faster house price growth over the year, increasing the need for larger mortgages.


House prices in Northern Ireland rose five times faster than the UK average for the past 12 months, while the last quarter saw house price inflation in Northern Ireland outpace the UK average ten fold. Scotland has also seen house price growth above the UK average.


The Northern Irish and Scottish housing markets are booming and, like their governments, have become increasingly devolved from the UK, concluded Fionnuala Earley, Nationwide's group economist.


Meanwhile, within England, the south has resumed its position as the focus of house price growth.


House price inflation in the south of England has now outpaced the north for the third successive quarter. This follows an extended period when the south lagged behind as buyers appeared to reach the limits of affordability.


London is also once again the city with the fastest house price growth, with inflation dampening in northern cities. Nationwide reports that this is also having a knock-on effect on the areas surrounding the capital.


There is a clear pattern of acceleration in house price growth in the south in the regions closest to London, compared with last year. At the same time there is clear deceleration in all of the regions in the north, concluded Ms Earley.


According to Nationwide's quarterly house price index, prices across the UK fell back sharply in the second quarter of the year from 2.2 per cent to 0.9 per cent. However, annual growth has remained stable.

The earliest recorded mention Italy comes from the Paleolithic era, which occurred over 200,000 years ago. The name Italy is said to have either come from the name Italus, who was one of the kings of the area, or the Homeric word that meant “bull”. By the third century BC, the Romans had taken over the entire area of the Mediterranean, which included Italy, and this boot-shaped land became part of the Roman Empire. This Roman era led to prosperity and peace for the land of Italy, and this was continued until around 400 AD.


The Germanic people became the rulers of Italy around 400 AD. They ruled for roughly 135 years until, in 535, Justinian reclaimed the area for Rome. For the next 1000 years, Italy became a fragmented group of city states, and the paths of the southern and northern parts of Italy differed greatly. In the northern area, the city states of Genoa, Florence, Milan and especially Venice prospered, both in areas of commerce and those of politics. The southern part of Italy, in areas such as Sicily, were taken over first by Muslims in the ninth century and then by the Normans in the 1050's. The areas of Sicily and Naples were filled with turmoil, but they also produced the climate that fed the Italian Renaissance movement where artists such as Michelangelo produced stunning works.


By the 1600's, the Hapsburgs had taken control of the area of Italy and were strong rulers of many city states. Their rule was opposed, however, and by 1861, Italy's smaller city states had united to form a full country known as Italy. Victor Immanuel II became the first king of Italy, and the country continued under a monarch until 1946, when a republic was developed.


Since that time, the republic has continued without interruption with one exception. In the 1930s, Mussolini became the elected President, but was for all intensive purposes a fascist dictator. As you know, he was deposed as Italy was defeated in World War II. Since that time, the country has remained a republic.

Firstly get yourself comfortable position whether suiting in a chair or lying on your bed and keep your back in straight position so nothing is effecting the air flow. Close your mouth and start to breathe in and out through your nose in a rapid fashion, try to breath about 2-3 inhalations a second for about 10 seconds. I know that at the start it will probably feel awkward and you will fill your muscle are having to work harder (this is good for strengthening your diaphragm) but the more you practice it you’ll find it easier to do and when you feel comfortable and gotten over the dizzy sensations you can then increase the length of time you do this for, add up to 5 seconds more onto this but don’t get carried away... it’s better to do small amounts over the day than try to do it over too long of a period.


This exercise works because it helps to increase the amount of oxygen coming into your body and give the body and muscles the needed the oxygen they have been needing. While this exercise is great in the short term you really need to work on your breathing technique overall to improve your energy levels right through the day.

It’s not everyday that you will come across a revolution in the making, but there you have it. By need the body cells convert the glycogen to glucose and use it as fuel. In this way insulin keeps the glucose level in the blood at a normal size.

Belgian malinois - protector of home family and livestock

The Belgian Malinois is a medium/large size dog that weighs between 40-80 lbs and is between 22" to 26" in height. Their name is pronounced MAL-in-wah. They are one of four Belgian Sheepdogs and are very work oriented and are protective of home, family and livestock.


The AKC recognizes the Belgian Malinois in the colors of fawn to mahogany to red with a black mask, ears and tips. Minimal white on the chest and feet is permitted. The short, smooth, straight and hard outer coat and dense undercoat require minimal grooming and are very easy to care for. Regular washing is not recommended as their coat has water proofing.


This intelligent, energetic, protective breed is still more of a working dog than companion. They have a strong drive to work. The Belgian Malinois makes an excellent farm dog, watchdog, police dog and herding dog because of their instinct to protect their family, home or whatever else they are trained to protect. They can make a great family pet as well. Early socialization is a must for children, other dogs and pets. In fact, they do best if they are socialized from the time they are born. They can be dominant towards other dogs. Early, consistent, firm but not harsh training is important. They are harder for a novice to train than someone who is more experienced. The Belgian Malinois has a lot of energy and loves to be on the go. They require a yard and plenty of room to run and play. They are not meant to be indoors all day. They enjoy being with their family and are not meant to be left alone for long periods of time as they will get bored and find some way to fill their time.


Originating in Belgium, the Belgian Malinois is one of four Belgian Sheepdogs. Their name comes from the Belgian city of Malines. It is recognize as its own breed by the AKC however, in some countries, all four Belgian Sheepdogs are considered to be one breed. The Belgian Malinois is popular in Belgium and is used for herding, police work and as a search and rescue dog.


A breed with such a strong work drive, protective instinct and high energy makes an excellent watchdog and farm dog. They do best with families that are active and will meet their work and exercise needs. An active, busy Belgian Malinois is a happy Belgian Malinois.


On achievement

If a comatose person were to earn an interest of 1 million USD annually on the sum paid to him as compensatory damages – would this be considered an achievement of his? To succeed to earn 1 million USD is universally judged to be an achievement. But to do so while comatose will almost as universally not be counted as one. It would seem that a person has to be both conscious and intelligent to have his achievements qualify.


Even these conditions, though necessary, are not sufficient. If a totally conscious (and reasonably intelligent) person were to accidentally unearth a treasure trove and thus be transformed into a multi-billionaire – his stumbling across a fortune will not qualify as an achievement. A lucky turn of events does not an achievement make. A person must be intent on achieving to have his deeds classified as achievements. Intention is a paramount criterion in the classification of events and actions, as any intensionalist philosopher will tell you.


Supposing a conscious and intelligent person has the intention to achieve a goal. He then engages in a series of absolutely random and unrelated actions, one of which yields the desired result. Will we then say that our person is an achiever?


Not at all. It is not enough to intend. One must proceed to produce a plan of action, which is directly derived from the overriding goal. Such a plan of action must be seen to be reasonable and pragmatic and leading – with great probability – to the achievement. In other words: the plan must involve a prognosis, a prediction, a forecast, which can be either verified or falsified. Attaining an achievement involves the construction of an ad-hoc mini theory. Reality has to be thoroughly surveyed, models constructed, one of them selected (on empirical or aesthetic grounds), a goal formulated, an experiment performed and a negative (failure) or positive (achievement) result obtained. Only if the prediction turns out to be correct can we speak of an achievement.


Our would-be achiever is thus burdened by a series of requirements. He must be conscious, must possess a well-formulated intention, must plan his steps towards the attainment of his goal, and must correctly predict the results of his actions.


But planning alone is not sufficient. One must carry out one's plan of action (from mere plan to actual action). An effort has to be seen to be invested (which must be commensurate with the achievement sought and with the qualities of the achiever). If a person consciously intends to obtain a university degree and constructs a plan of action, which involves bribing the professors into conferring one upon him – this will not be considered an achievement. To qualify as an achievement, a university degree entails a continuous and strenuous effort. Such an effort is commensurate with the desired result. If the person involved is gifted – less effort will be expected of him. The expected effort is modified to reflect the superior qualities of the achiever. Still, an effort, which is deemed to be inordinately or irregularly small (or big!) will annul the standing of the action as an achievement. Moreover, the effort invested must be seen to be continuous, part of an unbroken pattern, bounded and guided by a clearly defined, transparent plan of action and by a declared intention. Otherwise, the effort will be judged to be random, devoid of meaning, haphazard, arbitrary, capricious, etc. – which will erode the achievement status of the results of the actions. This, really, is the crux of the matter: the results are much less important than the coherent, directional, patterns of action. It is the pursuit that matters, the hunt more than the game and the game more than victory or gains. Serendipity cannot underlie an achievement.


These are the internal-epistemological-cognitive determinants as they are translated into action. But whether an event or action is an achievement or not also depends on the world itself, the substrate of the actions.


An achievement must bring about change. Changes occur or are reported to have occurred – as in the acquisition of knowledge or in mental therapy where we have no direct observational access to the events and we have to rely on testimonials. If they do not occur (or are not reported to have occurred) – there would be no meaning to the word achievement. In an entropic, stagnant world – no achievement is ever possible. Moreover: the mere occurrence of change is grossly inadequate. The change must be irreversible or, at least, induce irreversibility, or have irreversible effects. Consider Sisyphus: forever changing his environment (rolling that stone up the mountain slope). He is conscious, is possessed of intention, plans his actions and diligently and consistently carries them out. He is always successful at achieving his goals. Yet, his achievements are reversed by the spiteful gods. He is doomed to forever repeat his actions, thus rendering them meaningless. Meaning is linked to irreversible change, without it, it is not to be found. Sisyphean acts are meaningless and Sisyphus has no achievements to talk about.


Irreversibility is linked not only to meaning, but also to free will and to the lack of coercion or oppression. Sisyphus is not his own master. He is ruled by others. They have the power to reverse the results of his actions and, thus, to annul them altogether. If the fruits of our labour are at the mercy of others – we can never guarantee their irreversibility and, therefore, can never be sure to achieve anything. If we have no free will – we can have no real plans and intentions and if our actions are determined elsewhere – their results are not ours and nothing like achievement exists but in the form of self delusion.


We see that to amply judge the status of our actions and of their results, we must be aware of many incidental things. The context is critical: what were the circumstances, what could have been expected, what are the measures of planning and of intention, of effort and of perseverance which would have "normally" been called for, etc. Labelling a complex of actions and results "an achievement" requires social judgement and social recognition. Take breathing: no one considers this to be an achievement unless Stephen Hawking is involved. Society judges the fact that Hawking is still (mentally and sexually) alert to be an outstanding achievement. The sentence: "an invalid is breathing" would be categorized as an achievement only by informed members of a community and subject to the rules and the ethos of said community. It has no "objective" or ontological weight.


Events and actions are classified as achievements, in other words, as a result of value judgements within given historical, psychological and cultural contexts. Judgement has to be involved: are the actions and their results negative or positive in the said contexts. Genocide, for instance, would have not qualified as an achievement in the USA – but it would have in the ranks of the SS. Perhaps to find a definition of achievement which is independent of social context would be the first achievement to be considered as such anywhere, anytime, by everyone.

Hospitalists can you catch one in the hospital

It's an emerging trend, and one that many patients find confusing and uncomfortable: their primary doctor doesn't visit them in the hospital any more and doesn't manage their hospital care.


Primary care doctors are increasingly turning the care of their hospitalized patients over to specialists called "hospitalists."


The hospitalist is a hospital-based doctor who does not see patients in an office-based practice. He or she manages the care of patients only while they are in the hospital, turning them back over to their regular physicians when they are discharged. During the time a patient is in the hospital the hospitalist is responsible for all decisions about a patient's care.


Advantages of Hospitalists


The hospitalist usually knows the hospital, and hospital politics, very well. This often enables the hospitalist to cut through red tape and "make things happen" more efficiently than office-based physicians.


Hospitalists are more readily available to respond to emergencies in the hospital. Nurses and other care staff can usually reach a hospitalist more rapidly than an office-based physician, especially on evenings and weekends.


Continuity of care within the hospital is often better. When primary care physicians manage inpatient hospital care, the patient is often actually seen by more doctors, as doctors in larger practices often take turns seeing all of the practice's hospitalized patients.


Hospitalists are usually more accessible to family members. Families don't have to try to "catch" the doctor in the wee hours of the morning or late in the evening when he or she is making hospital rounds outside of office hours.


Disadvantages of Using a Hospitalist


The biggest disadvantage to the movement toward hospitalists is the loss of continuity of care between the primary physician and the hospital. The hospitalist has no previous knowledge of his new patient. If communication between the primary care physician and the hospitalist is poor, it falls to the patient and the family to fill in the gaps.


When a patient is discharged from the hospital the hospitalist relinquishes care back to the PCP. If communication has not been good, the primary care physician often has little knowledge of what the patient experienced in the hospital. Records are frequently slow to follow the patient, so on the first follow-up visit the office-based doctor may have scant information.


Without adequate information the PCP frequently makes adjustments to treatment plans and medications that are counter to the treatment plans initiated in the hospital.


How To Work With A Hospitalist


Be prepared. If your trip to the hospital is pre-planned, talk to your primary doctor about the hospitalists in your chosen hospital. Find out which hospitalist communicates best with your doctor, and who your doctor prefers to work with. If possible, ask your doctor to pre-arrange that this hospitalist will be in charge of your care while you're in the hospital.


Arrive Armed: Never assume that things will go as planned. You may not feel well, and you will be under stress. The chosen hospitalist may be on vacation, out ill, or unavailable. Take a synopsis of your medical history, including the results of all recent tests, with you to the hospital. Whenever possible have someone stay with you in the hospital until you have met your doctor and given him or her all the information you have. Have that person take notes, including the names of all your hospital caregivers, their contact information (phone and pager), and the location of their offices in the hospital.


Sign a Release: If you can, make sure to sign a release of information form when you are admitted. This will give the hospitalist and everyone else on your medical team permission to discuss your care with the person you appoint. Even if you have already given that person your Power of Attorney for Health Care, signing a permission to release information is a good idea. You might not be so ill that your Power of Attorney for Health Care goes into effect. In that case, without authority to release information, you might find the staff is less than willing to share information with the person staying with you.

5. Populate your database as quickly as possible. People follow other people when they join dating sites and if your site looks like it hasn't got many members, then people will give it a wide berth. When you launch, it's well worth you doing a limited email marketing campaign which offers a free subscription to founder members. Remember, the hardest part of creating a successful online dating business is to get your first 100 registrants - you want them there in week one and the quickest way to do that is to offer a freebie to your first sign-ups. You're not losing money - these are the most important members and without them your business will not get past first base.


6. Spend your marketing budget wisely. You don't have to throw money at a dating site in order to promote it effectively - you just have to be smart. Affiliates are third party web-sites which refer traffic to your business on a revenue sharing basis. And you should also avoid generic search-terms when you instigate pay-per-click campaigns on the major engines. Keywords like 'dating' and 'online dating' are expensive to bid on and convert less well than niche terms which are more applicable to the actual concept of your site. If, for example, you are running a dating service for singles living in Chicago, keywords like 'Chicago dating' or 'singles in Chicago' will be relatively cheap to bid on and will yield a far better conversion rate than the more general terms.


7. Offer good customer service. Remember, your members are real people many of whom will not be as computer savvy as you are and when any of them contact the site with a problem (a forgotten password or log-in difficulties are usually the most common), you should endeavor to respond as soon as you are able. An unanswered message is the easiest way to actually lose a member so don't ignore or put off answering the mails that you will inevitably receive.


8. Communicate with your members. I've left this point until last but it it just as important as anything I've mentioned above. A weekly newsletter to your database will encourage people to log-in to their accounts on a regular basis (particularly if they are able to click through directly from the email to the site without having to log-in) and will also help to foster an emotional bond between your business and it's paying customers. Every business to consumer online entity should have a human face and site owners who ignore this do so at their peril.

Knowing the risk of gastric bypass

There is more to gastric bypass than merely controlling stomach size. During gastric bypass surgery, a large portion of the patients stomach is stapled together, leaving a tiny pouch.


When you get your gastric bypass surgery you will have a new stomach that only holds about three tablespoons of food at a time. With your stomach pouch reduced to the size of a walnut, you'll need to follow a gastric bypass diet.


The surgeon does not remove any part of the stomach or other tissue while doing the gastric bypass operation. This operation is the most common gastric bypass procedure performed. Due to the changes in digestion after gastric bypass surgery, patients may develop such nutritional deficiencies as anemia, osteoporosis, and metabolic bone disease.


The gastric bypass operation reduces the progression and mortality of non-insulin-dependent diabetes mellitus. However, this complication only occurs to a very small degree in the gastric bypass operation. One of biggest advancements in the gastric bypass operation has been the technique used to enter into the abdomen through the laparoscopic approach.


Gastric bypass surgery creates dramatic changes in the size and shape of the stomach. Barely 16 months after gastric bypass surgery, which reduced your stomach to the size of a hard-boiled egg, a patient is able to shed 185 pounds.

On the horizon future fuels

Between elevated gasoline prices (which show every indication of staying that way) and the current concern about the environmental impacts of vehicle emissions, you've probably been hearing a lot of discussion about the use of "alternative fuels." It sounds like a good idea, you think...but do you know what alternative fuels are actually in the running? Here's the shortlist of the alternative fuels to keep an eye on, and a major advantage and disadvantage for each:


Electric


Electricity is a source of power that we already produce, store, and use. To power an electric vehicle (EV), electricity is produced by a power plant or other standard means and is then stored in the battery that will operate the vehicle.


Pro: A vehicle powered solely by electricity produces no emissions.


Con: In current models, batteries need to be recharged frequently.


Biodiesel


Biodiesel is a fuel that is made from vegetable oils and animal fats through a chemical process called esterification. When the oils and fats are mixed with alcohol, their molecules break down into methylesters, which are then used as fuel, and a waste product of glycerin. It can be used in some diesel vehicles with very little, if any, modification to the existing engine.


Pro: Potential fuel sources vary from grain crops to used cooking grease.


Con: Biodiesel thickens and freezes at low temperatures.


Ethanol


Ethanol is a type of alcohol that is created through the fermentation of substances with high starch or sugar contents, such as corn or sugar cane. In this process, yeast bacteria consume the sugars and starches and produce carbon dioxide and ethanol as by-products. The use of chemical catalysts can be used to make the process faster and more productive.


Pro: Ethanol can be produced from renewable agricultural resources.


Con: Contains less energy than the same amount of gasoline.


Hydrogen Fuel Cells


Hydrogen fuels cells harness the electrochemical reaction between hydrogen and oxygen. The bonding of hydrogen and oxygen atoms results in two products: energy, which is then stored as power in the cell, and—you guessed it: water.


Pro: Hydrogen engines are typically more efficient than gasoline engines.


Con: Current vehicle designs can only store enough hydrogen for short-range travel.


At first, you’ll most likely see these fuels used in combination with current gasoline-engine technology. Hybrid electric vehicles (HEVs) utilize both a standard gasoline engine and a rechargeable battery, and are already offered by several car manufacturers. Hybrid vehicles with a gasoline engine and a hydrogen fuel cell are currently in development, and are estimated to hit the market in 2008 to 2009. Vehicles that are designed or modified to run on mixtures of gasoline and biodiesel or ethanol show promise, and are already in use.


While some types of alternative-fuel vehicles aren’t yet readily available to the public, keep your eyes open—a combination of research, public encouragement, and lively market competition brings that day ever closer. Honda and Toyota, the first automotive manufacturers to market HEVs in the United States, have already been joined by a handful of others…and the rest are likely to follow suit.
